How does a lower eyelid lift work?
Depending on the individual situation, treatment is either minimally invasive or surgical:
Transconjunctive technique
An approach via the conjunctiva (from the inside) enables fat to be removed or redistributed – without an external scar.
Depending on the findings, this technique can be combined with a suspension of the eyelid edge (canthopexy) and/or fine skin removal via an infraciliary incision. In this way, even slight excess skin can be gently corrected.
Transcutaneous, infraciliary technique
With this method, the fine incision is made just below the lash line.
It allows excess skin to be removed, the eyelid muscle to be tightened and – if necessary – fatty tissue to be redistributed in order to achieve a harmonious result.
The eyelid muscle is usually also tightened to improve the durability of the result.
The fine scar lies in the natural crease of the eyelid and is usually barely visible after healing.
This technique is mainly used if there is a relevant excess of skin in addition to the fat pads – in many cases, however, the transconjunctival technique can be a gentler alternative, as it does not require an external scar and the muscle is not weakened.
Duration: approx. 45-60 minutes.
Are there alternatives to surgery?
Yes, if it is mild or if there is no sagging of the skin:
- Hyaluronic acid filler for gentle smoothing of the tear trough
- Autologous fat transfer (lipofilling) for volume augmentation with the body’s own tissue
These methods are well suited for deep-set dark circles or volume loss – however, surgical tightening remains the most effective option for real bags under the eyes with excess skin.

Is the operation painful?
No – post-operative pain is usually minimal and can be well controlled with simple painkillers.
Will a scar remain?
This depends on the technique used:
- With the transconjunctival method (internal approach via the conjunctiva), there is no visible external scar as the incision is made inside the eyelid. This technique can be combined – without weakening the circular muscle – with an incision below the lash line to remove any excess skin.
- With the transcutaneous, infraciliary technique (external access below the lash line), a fine incision is made directly on the edge of the eyelid. This lies in the natural crease of the eyelid and usually heals so inconspicuously that it is barely visible.
How long does the result last?
Usually many years – often a single procedure is sufficient for a permanently rejuvenated appearance.
How much does a lower eyelid lift cost?

Can the skin quality also be improved?
